What Types of Support Are Available?
Home support can be short or long term and is adapted as your needs change. For those coming home from hospital or dealing with an acute health challenge, staff may be there for you around the clock then, as you become stronger, we lessen the support. In other situations, a brief visit - say an hour each week - is all that is needed to support you with things that may become challenging as you age or deal with health concerns. Many clients find that a few hours a month enables them to live well in their own homes.
Services may include:
- bath or shower assist, dressing and laundry
- wound care
- medication support
- application of compression stockings, braces or prosthetics
- grocery shopping or meal preparation
- housekeeping (select geographical areas)
- palliative care
- all levels of nursing care
- dementia care and respite
- transportation and support with activities of daily living
- assistance upon discharge from hospital or, companionship for those in hospital or care homes
- diabetic care and teaching

Becoming a Client
We receive referrals from trustees, insurance representatives, physicians, hospital discharge planners, and of course, individuals and families themselves. Upon request, a client care manager meets with the client and/or family to answer questions and provide an in home assessment at no cost to you. We ask about current and past health concerns, existing supports, what is now a challenge or what may be a worry. These needs are then balanced with what a person prefers in the way of support. Care is then tailored to each individual's preferred lifestyle, habits, comforts, preferences and health needs.
To request additional information or to have a Nursing/Care Manager visit you or a family member, call toll free 1-866-861-3446 or 250-861-3446(Kelowna), or 250-492-6161(Penticton). There is no charge for this information or service.
If, following this initial meeting, you wish to receive support, the Care Manager would then create a Plan of Care, which is individualized to your needs and preferences.
